Contact Nest & Neighbor Care to discuss your loved one’s needs, your situation, and the level of support required. This initial conversation helps us determine the most appropriate level of care.
2. Schedule an in-home assessment
Following the initial call, we will schedule an in-home assessment at your convenience. This allows us to better understand your loved one’s needs, environment, and preferences.
3. Complete the intake process together
During the in-home visit, we will complete the intake process together, ensuring everything is clear, comfortable, and tailored to your family.
4. Review and sign documents in the client portal
After your assessment, you will receive access to your client portal where you can review and sign your service agreement and any additional care-related documents.
5. Care plan creation and service scheduling
Our team will finalize your personalized care plan and coordinate caregiver scheduling so services can begin smoothly.
